What it is

Vitamin B12 is essential for red blood cell formation, neurologic function, and DNA synthesis. An injection may be used when laboratory results, diet, or absorption concerns indicate a need.

How it works

Intramuscular B12 bypasses gastrointestinal absorption and raises available cobalamin. The underlying cause of a deficiency, such as diet, medication use, or malabsorption, should be evaluated. Treatment response is checked with symptoms and appropriate laboratory testing.

Mechanism in depth

Vitamin B12 (cyanocobalamin, methylcobalamin, or hydroxocobalamin) is a cofactor for methylmalonyl-CoA mutase and methionine synthase. It is essential for red blood cell formation, myelin synthesis, and DNA production. Oral B12 requires intrinsic factor for gut absorption, which is impaired in some adults; injections bypass this bottleneck.

What outcomes to expect

For patients with documented deficiency, energy and cognitive complaints often improve over two to eight weeks. In patients with borderline or normal levels, subjective benefits vary. Neurologic recovery from prolonged deficiency can take months and may not be complete.

Common side effects

Injection site soreness, mild bruising, and rare allergic reaction. B12 has an excellent safety profile at typical doses.

Monitoring schedule

Baseline B12 and folate labs. Follow-up labs at eight to twelve weeks to confirm response. Ongoing dosing frequency depends on cause of deficiency — patients with pernicious anemia or malabsorption typically require lifelong periodic injections.

Not appropriate if

Not appropriate if you have a known cobalt or cobalamin allergy, active Leber's hereditary optic neuropathy, or if pregnancy is possible without physician review.
