This Telehealth Informed Consent describes what to expect from telehealth care provided through GPLX (Genesis Longevity & Wellness) by licensed medical providers affiliated with Elite Care Health, an independent physician group. By submitting an intake, holding a consultation, or accepting a treatment plan through the Services, you acknowledge and agree to the terms below.

1. What Telehealth Is

Telehealth is the delivery of healthcare services using interactive audio, video, secure messaging, and asynchronous review of intake responses, photographs, and lab results. Telehealth allows you to receive care from a licensed medical provider without an in-person visit.

2. Who Provides Your Care

Your care is provided by a licensed medical provider affiliated with Elite Care Health. Your Provider is licensed in the state where you reside at the time of the encounter. GPLX itself does not practice medicine; GPLX provides the technology, membership, and support that connect you with your Provider.

3. How Your Care Will Be Reviewed

Your intake is reviewed by a licensed medical provider. Depending on your health history, goals, treatment request, and applicable requirements, the Provider may request additional information, lab work, or a synchronous consultation before making a treatment decision. No prescription is guaranteed. Completing an intake or selecting a program does not guarantee treatment. Prescription medications are only provided when a licensed medical provider determines they are clinically appropriate.

4. Benefits of Telehealth

  • Access to licensed medical providers without travel;
  • Convenient scheduling and secure messaging;
  • Continuity of care across a defined treatment program;
  • Coordinated prescription fulfillment through Rx Ave Health.

5. Limitations and Risks of Telehealth

Telehealth is not equivalent to an in-person visit. Limitations and risks include, but are not limited to:

  • The Provider does not physically examine you, which may limit clinical assessment;
  • Technology failures (internet outages, poor video quality, dropped connections) may interrupt care;
  • Information you provide may be incomplete or inaccurate and could lead to a less-than-ideal treatment decision;
  • In rare cases, electronic transmission of information could be intercepted or breached despite security safeguards;
  • Telehealth may not be appropriate for your condition, in which case your Provider will refer you to in-person care.

6. Not for Emergencies

GPLX is not for medical emergencies. If you are experiencing a medical emergency — including chest pain, difficulty breathing, stroke symptoms, severe bleeding, or thoughts of harming yourself or others — call 911 immediately or go to the nearest emergency room. GPLX does not provide primary care, emergency care, or urgent care services.

7. Compounded Medications

Some treatments prescribed through the Services may be compounded medications prepared specifically for you by Rx Ave Health. Compounded medications are not FDA-approved. The FDA does not evaluate compounded medications for safety, effectiveness, or quality. Your Provider will discuss the risks and benefits of any compounded medication before it is prescribed.

8. Confidentiality & Records

Your telehealth visits, messages, and treatment records are maintained by Elite Care Health under HIPAA and applicable state law. See our Notice of Privacy Practices for how PHI is used and disclosed, and our Privacy Policy for information collected through the website and membership platform.

9. Your Rights

  • You may withdraw this consent at any time by writing to support@gplxlife.com. Withdrawal will not affect care already provided;
  • You have the right to decline any recommended treatment;
  • You have the right to choose an in-person visit with a different provider instead of telehealth;
  • You have the right to request a copy of your medical record and to request corrections;
  • You have the right to information about the identity and credentials of your Provider before treatment.

10. Your Acknowledgment

By submitting an intake, holding a consultation, or accepting a treatment plan through GPLX, you acknowledge that:

  • You have read this Telehealth Informed Consent and had an opportunity to ask questions;
  • You understand the benefits, limitations, and risks of telehealth;
  • You understand that no prescription is guaranteed and that your intake will be reviewed by a licensed medical provider;
  • You will provide accurate and complete health information;
  • You will not use the Services for emergencies;
  • You are at least 18 years old and a U.S. resident.
