What it is

Free testosterone is the fraction of testosterone not bound to carrier proteins. It is the portion immediately available to tissues.

Why it matters

Free testosterone often correlates better with symptoms than total testosterone, especially when SHBG is high or low. Two patients with the same total value can have very different free testosterone.

GPLX generally targets a free testosterone range in the upper half of the lab reference range in symptomatic men. Ranges vary by assay method. Interpretation belongs with your physician.

What abnormal values can suggest

If low

A low free testosterone value can suggest hypogonadism, elevated SHBG, hypothyroidism, or medications that shift binding proteins.

A high free testosterone value can suggest exogenous testosterone use, PCOS in women, or a suppressed SHBG state.
